Spring North are inviting VCFSE organisation to deliver community-based care coordination aimed at reducing avoidable hospital admissions across Lancaster, Morecambe & Carnforth.

This project is funded by the Urgent & Emergency Care (UEC) Capacity Grant and supported by the Lancashire and South Cumbria Integrated Care Board (ICB) and the LSC VCFSE Alliance, which champions the role of the VCFSE sector in creating a stronger, more connected health and care system. 

Over a 6-month period, the project will provide non-clinical, wraparound support to people of all ages who are at risk of needing urgent or emergency care. The aim is to help individuals stay safe, connected, and well in their communities, easing pressure on frontline services. 

Organisations will work closely with the NHS and community partners to strengthen coordination, link people to local services, and offer early, person-centred support that prevents crisis.

  • Available funding - Up to £100,000
  • Delivery period - 6 months (Sept 2025 - Mar 2026)
  • Deadline - Friday 15th August 2025
