Have you used health services in Chorley, South Ribble or Greater Preston in the last three years? If yes, then the Our Health Our Care team would like to hear from you.

Health services in our area may be changing. To help us design services for the future, we want to hear the positive and negative experiences of patients who have used health services in Chorley, South Ribble or Greater Preston. We are particularly interested in hearing from those who have accessed any services in the following areas:

Surgery: Have you been admitted to hospital for planned surgery following a referral?

Urgent and emergency care: Have you had to spend time in A&E or needed emergency care for a life threatening condition?

Critical care: Have you received care when critically ill? For example, in an intensive care or high dependency unit bed?

Acute medicine: Have you received treatment in a hospital bed for the first 24 to 72 hours of an emergency admission?

Speciality medicine: Have you received care from medical speciality 72 hours after being admitted to hospital?.

We would like to invite patients/ relatives and/or carers who have used the services to join local senior doctors and nurses at a workshop to discuss and directly influence future models of care. If you would like to attend, please choose from the following sessions (both cover the same content so attendance at each is not required)

Monday 25th June 2018 or Tuesday 26th June 2018
