Bay-wide Integrated Musculoskeletal Service (iMSK)
In 2017, the iMSK service was created for South Cumbria and has since expanded to include the whole of Morecambe Bay. The community-based service offers interventions including physiotherapy and ensures that patients receive the best out of hospital management, decision support, lifestyle and exercise advice.
The vision for the MBRN is simple – to dramatically improve care for patients with respiratory disease in Morecambe Bay with a seamless, integrated care pathway. Through the MBRN model patients have one self-management plan from all teams, quicker access to specialist investigations and therapies, fewer appointments at hospital, better care with their GP and consistent information from everyone.
Outpatients is identified as a key area of change for Bay Health & Care Partners, and there are many opportunities for transforming outpatients, some of which may involve technology. The programme of work is scheduled over a five year period. It will support the delivery of health services that reflect national and local Lancashire and South Cumbria Integrated Care Service (ICS) priorities.
Primary Care Networks
Primary Care Networks (PCNs) are collections of individual GP practices which remain independently responsible for delivery of their contracts; they’re not new organisations. PCNs are created by a practice (or practices) signing up to this new aspect of the GP contract, which is discretionary and offered to all practices in England. In Morecambe Bay there are eight PCNs of various sizes. Formed in mid-2019, all but one practice have taken on the PCN work within BHCP.
