Population Health Improvement

  • Deliver population health plan

BHCPs Vision

To see a network of communities across Morecambe Bay enjoying great physical, mental and emotional wellbeing, supported by a health and care system that is recognised as being as good as it gets.

Our objectives:

To achieve our vision we will deliver our “triple aim”:

  • Better Health - we will improve population health and wellbeing and reduce health inequalities
  • Better Care - we will improve individual outcomes, quality and experience of care
  • Delivered Sustainably - we will create an environment for motivated, happy staff and achieve our control total

National priorities - emergency care, cancer & mental health

The Actions we will take. 

We will:

  • Improve delivery of the cancer pathways to ensure we consistently achieve national cancer standards.
  • Improve delivery of mental health care & learning disability services, particularly by: re-designing child and adolescent services through the nationally recognised ‘Thrive’ model; establishing a single integrated Mental Health & LD provider across the Bay; and by increasing community services to prevent the need for unplanned acute care.
  • Improve emergency care services to ensure we meet the national urgent care standards by 2020/21.

The way we will measure success 

As a sign of succes, we will see: 

  • 85% of patients receiving first treatment for cancer within 62 days of their GP referral
  •  An increase to 32% of children and young people with a diagnosable need accessing CAMHS services.
  • Elimination of 12 hour breaches in A&E units and achievement of the Bay trajectory for the A&E 4 hour standard.

Financial Recovery 

The Actions we will take. 

We will:

  • Achieve our control total by developing and delivering our financial plans whilst maintaining clinically safe and effective care. We will use national NHS benchmarking tools (such as Model Hospital and RightCare) to help us improve performance against peers by:
  • Transforming care through improved pathways to improve outcomes and cost effectiveness.
  • Delivering cost improvement programmes where we are an outlier against national benchmarks.
  • Using technology and digital skills to ensure more cost effective and improved care delivery.

The way we will measure success 

As a sign of succes, we will see:

  • Achievement of our financial control total.
  • An increase in care being delivered through digital solutions.

Build the Morecambe Bay Integrated Care Partnership (ICP) and the Lancashire and South Cumbria Integrated Care System (ICS)

The Actions we will take. 

We will:

  • Continue to build the Morecambe Bay ICP in order to improve delivery our triple aim of better health, better care, delivered sustainably
  • Continue to be active partners in the development and delivery of ICS priorities (such as stroke and Children’s care)
  • Develop a positive work culture so that staff want to work in the Bay and are motivated to deliver excellent care

The way we will measure success 

As a sign of succes, we will see:

  • Delivery of the actions and targets above
  • A positive assessment of ICP/ICS developments (as measured through regulatory assessments and stakeholder surveys)
  • An increase in staff who would recommend working in Morecambe Bay to friends and colleagues
