Bay Health and Care Partners (BHCP) priorities for 2019/20 were focused around four key areas:

  • Population health

  • Integrated Care Communities and primary care

  • National priorities

  • Financial recovery.

Below is a summary of key achievements for 2019/20: 

  • BHCP supported our Integrated Care Communities to deliver a number of community-based health events such as Lancaster Healthfest.

  • We joined the National Atrial Fibrillation programme: meaning that patients on GP registers who are at risk of a stroke and would benefit from anti-coagulation are being identified with the aim of reducing strokes and unnecessary hospital admissions.

  • We developed a diabetes pathway aimed at drastically reducing the number of people with type 2 diabetes and supporting those who have diabetes to manage their own condition better.

  • Millom Integrated Care Community supported peer-led community-based Mental Health support.

  • The Integrated Care Communities ran regular Farmers Health clinics at local auction marts across Lancashire and South Cumbria. 

  • In emergency care, BHCP achieved a 50% reduction in patients medically fit for discharge waiting in acute hospital beds, releasing 50 beds cross-bay.

  • We implemented a new cancer pathway aimed at speeding up cancer diagnosis and treatment.
