Meeting: Primary & Community Care System Delivery Board

Key Decisions and Actions:

The Board heard a number of updates within the first meeting of the new year. Discussions were held around the continued pressures within the system caused by a further lockdown. Members were reminded of the importance of this Board, in that the support of discharges from hospital is not the only priority, it is to ensure an offer of ‘wrapped around’ care is available to those most vulnerable and keeping people at home, out of hospital.

Work continues to progress within the CATCH sub-group, though concerns were raised around funding going into a new financial year. The Board heard of the ICP Priority Setting process currently in development. This process will ensure that our offer is consistent and our efforts are not de-railed.

Members received a presentation from Louise Taylor around a project that has been developed of which aims to create a clear vision for the Central Lancashire Integrated Care Partnership as to how the ICP will work together as public services to improve outcomes for the communities in Chorley, South Ribble and Preston. It will consider what an integrated approach would look like in a range of settings, from within our communities, to ensuring that people are living well and at points of access to services.

The key decisions taken from this meeting included;

  • Funding for LSCFT which supports the step up offer to be confirmed.
  • The ICP Priority Setting Process to be presented to the Board upon completion.
