Meeting: Primary & Community Care System Delivery Board

Key Decisions and Actions:

In order to prepare for the Deep Dive session scheduled with the Clinical and Professional Forum, Shashidhar Khandavalli, Clinical Lead for the Determinants of Health System Delivery Board attended this month’s Board meeting to provide a presentation around the Disease Profile that have been developed by the Determinants of Health SDB and approved by the Clinical and Professional Forum.

Members were left enthused and energised following the presentation and were eager to understand how they could each play a role in tackling inequalities within Central Lancashire in the context of progressing improvement in Intermediate care, primarily to help keep people stable and at home .

The key decisions taken from this meeting included;

• Work will go ahead to document an approach of how the work on Intermediate Care facilitates an approach driven by inequalities. This will be picked up as part of the Continuous Improvement as the inequalities agenda has not yet been informed as part of the pathway redesign.

• A sub-group will meet prior to the next SDB meeting to continue discussions with Shashidhar Khandavalli to further thinking around ways of which this SDB can adopt said approach.

• Members agreed to make a commitment to review and understand our population health management data, to understand the segments within the data and to prioritise working with our ‘at risk’ groups to reduce the health inequalities and to improve life expectancy.
