What is Healthier Lancashire and South Cumbria?
Our out of hospital strategy is well underway with our vision for joined up care, Building for the Future. We align to Lancashire in much of this work, but our plans and developments must also consider our patient flows through hospital, which is why we are also an Associate Member of the Cheshire and Merseyside STP.
There are many areas of focus for us and five priorities have been identified (detailed in our LDP). A new provider will be delivering local community health services and urgent care services (from 1 May 2017). This is integral to our future plans and move towards full integration and new models of care.
Our Well Skelmersdale programme continues to build momentum helping to encourage a happy healthier community, while minimising existing health inequalities. Musculoskeletal service improvements remains an area of focus and one of our Right Care priorities. We have now moved to level 3 primary care co-commissioning, a significant shift further towards our primary care transformation plans. Working closely with partners on the hospital sustainability plans also remain a core priority for us. Mental health is also a key element of our LDP, as we aim to improve local services in reflection of our local patient need.
Our Local Delivery Plan is simply what we strove for locally in 2016/17 and what we hope to deliver in 2017/18. This involves monumental partner working. Not only with our Lancashire wide colleagues, but also our local partners such as CCG colleagues in Southport and Formby, West Lancashire Borough Council, Virgin Care, Southport & Ormskirk Hospital NHS Trust, Lancashire Care NHS Foundation Trust, social care, our active voluntary community and faith sector, GP membership, patients, carers, our staff and many more.
Across Lancashire and South Cumbria, we face some real challenges which mean we can't carry on delivering health and social services the way we do now. Changes to community nursing in West Lancashire is an example of new ways of working that is improving experiences of patients.
